A volunteer sign up form for your charity or community event
Build a volunteer sign up form with shifts, skills, availability and an emergency contact, then publish it at a link and track who has signed up.
Register for the event
Saturday 14 June, 10:00 to 16:00. Places are limited.
This is a real form: answer it the way a visitor would. Nothing is saved.
Use this template Describe yours in chatOpen full screenMore to start from: all form, quiz and course templates. Nothing is published until you press Publish, and every answer lands in your account with charts and a CSV.
Volunteer sign-ups usually arrive as a mix of emails, texts and a paper list, and the coordinator ends up chasing availability one person at a time. This page builds a sign-up form for your shifts, skills and emergency contacts, then gives you a link, a results page and a CSV. The live sample above is a general event registration; the chat adapts it to your charity or event.
What you get
Questions that appear only when they apply
Show the first aid certificate question only to people who ticked first aid, and the parent sign-off only when age is under 18. Volunteers see a shorter form, and you get answers you can actually use.
Logic and pages
Show a follow-up only when it applies, split a long form into pages, or show one question per screen.
Results you can read
A chart for every question, averages and NPS for scales, the order of a ranking, a table of every answer and a CSV.
How it works
- Step 1
Try the sample above, then press Use this template, or describe your own form to the chat.
- Step 2
Change any question by hand or ask the assistant ("add a question about price", "translate it to Spanish"), with a live preview.
- Step 3
Publish, share the link or QR code, and read the answers as they arrive. You get an email for each one.
Who uses it
Food bank or pantry coordinator
Run a shift choice question with a response limit, then check the results page to see which Saturday slots still need people before the next sorting day.
Community event organiser
Ask volunteers to rank preferred roles like setup, stewarding or first aid tent, and get an email for each response so you can build the rota the same week.
Youth or sports club recruiting helpers
Include an under-18 parent sign-off and an emergency contact, then export the sign-ups as a CSV for the club's records.
Questions
How do I stop too many people signing up?
The form has a response limit that closes it once you have enough sign-ups. It closes the whole form, not one shift, so if shifts fill at different rates, check the charts on the results page and ask the chat to change or remove the options that are full.
What should I ask for in an emergency contact section?
Usually a name, relationship and phone number, and only if you need it for the activity. Health and children topics carry legal duties, so you are responsible for what you collect and should not ask for more than you need.
Can volunteers under 18 sign up?
Yes, if your organisation allows it. Add an age question and a parent or guardian consent step that only appears for under-18s. Check the rules that apply to your activity, as the form does not decide them for you.
Can I get the sign-ups into a spreadsheet?
Yes. The results page shows the answers with charts, and you can download them as a CSV to sort by shift or skill in any spreadsheet program.
Do I get told when someone signs up?
You can set the form to email you for each response. Volunteers see your thank-you message on screen after they submit, so tell the chat what it should say, such as when you will contact them about their shift.
