How to make a wedding RSVP form that gets the headcount, meals and allergies right
Build a wedding RSVP form with attendance, guest count, meal choice, allergies and a song request. Meal questions skip guests who decline.
Are you coming?
This is a real form: answer it the way a visitor would. Nothing is saved.
Use this template Describe yours in chatOpen full screenMore to start from: all form, quiz and course templates. Nothing is published until you press Publish, and every answer lands in your account with charts and a CSV.
Most wedding RSVP cards come back with a missing name, a vague 'two' and no meal choice, and the caterer has to chase each couple by phone. This page builds a form for your guest list: attendance, party size, named guests, main course, allergies and a song request. The live sample above is a general one; the chat rewrites it for your venue, menu and guest list.
What you get
Questions that appear only when they apply
Logic lets you show the meal and allergy questions only to guests who answer yes to attending, and the plus-one name box only when the party size is more than one. Guests who decline finish in a few seconds, and the caterer sees only the people who need a plate.
Logic and pages
Show a follow-up only when it applies, split a long form into pages, or show one question per screen.
Results you can read
A chart for every question, averages and NPS for scales, the order of a ranking, a table of every answer and a CSV.
How it works
- Step 1
Try the sample above, then press Use this template, or describe your own form to the chat.
- Step 2
Change any question by hand or ask the assistant ("add a question about price", "translate it to Spanish"), with a live preview.
- Step 3
Publish, share the link or QR code, and read the answers as they arrive. You get an email for each one.
Who uses it
Couples with a fixed-price venue menu
Set the meal options to match the menu you've agreed with the caterer, then read the results page for a count of each main course before the final numbers are due.
Families with guests who are not on email
Send the link by text or print the QR code on a card for older relatives. The form works on a phone, and one question shows per screen if that is easier to follow.
Couples planning a plus-one policy
Ask for each guest's full name only where the party has more than one person, so the place cards and the seating plan match what the names actually are.
Questions
Can the form ask only the people who are coming to answer the meal question?
Yes. Set the meal and allergy questions to appear only when the answer to 'Will you attend?' is yes. Guests who decline won't see them.
How do I get the numbers for the caterer?
Each response is listed on the results page with charts for attendance and meal choice, and you can download everything as a CSV. An email can also arrive for each response, so you can track numbers as they come in.
Should I ask about allergies?
Asking is useful, because the caterer needs to know. Keep the question short and specific to food, and only collect what the venue needs to plan safely. You are responsible for what you collect and how you store it, so don't add health questions beyond that.
Can I close the form on a set date?
You can set a response limit, and you can change the thank-you message to say the replies are closed. Share the date in your invitation so guests know when to answer.
Can I change the questions after guests have started replying?
You can edit the form by chat at any time. Before you change a question that guests have already answered, check the results page to see how the new wording will read against the answers you have so far.
