Make a club registration form for your student society
Build a membership sign-up form for your student club or society: name, year, interests, skills, how members found you, and consent.
Register for the event
Saturday 14 June, 10:00 to 16:00. Places are limited.
This is a real form: answer it the way a visitor would. Nothing is saved.
Use this template Describe yours in chatOpen full screenMore to start from: all form, quiz and course templates. Nothing is published until you press Publish, and every answer lands in your account with charts and a CSV.
Club sign-ups usually arrive as a mix of emails, paper sheets and chat messages, and the committee ends up retyping names just to know who is coming to the first session. The live sample above is a general event registration form; the chat adapts it to your society, so the questions match your members and what your committee needs to know. You publish it at one link and read the answers as they come in.
What you get
Follow-up questions that appear only when relevant
Logic shows a question only when an earlier answer matches. A new member who says they have done the activity before sees a question about their level, and everyone else skips it. The form stays short for people with no experience and still gets the detail you need from those who have it.
Logic and pages
Show a follow-up only when it applies, split a long form into pages, or show one question per screen.
Results you can read
A chart for every question, averages and NPS for scales, the order of a ranking, a table of every answer and a CSV.
How it works
- Step 1
Try the sample above, then press Use this template, or describe your own form to the chat.
- Step 2
Change any question by hand or ask the assistant ("add a question about price", "translate it to Spanish"), with a live preview.
- Step 3
Publish, share the link or QR code, and read the answers as they arrive. You get an email for each one.
Who uses it
Society secretary at the freshers' fair
Put one link on a QR code at the stall and collect sign-ups without carrying paper sheets. The results page shows how many people joined and which sessions they picked, so you know how many to plan for before the first week.
Treasurer or membership officer
Ask for the year of study and email, and keep the form free of anything you don't need. Use the CSV export to build your membership list, and send the consent-based mailing only to people who ticked the box.
Captain of a sports or outdoors club
Add a question on experience and equipment hire, with follow-ups for anyone who wants to borrow kit. Use a response limit to cap the trial session at the number of places you can safely run.
Questions
Can I close the form once we reach our limit for a trial session?
Yes. Set a response limit on the form so it stops accepting sign-ups when the number is reached, and you can show a short message in its place. This is useful for trips, workshops and anything with a fixed number of seats.
Can the form take membership fees?
No. The form has no payment step. You can ask whether someone plans to pay the membership fee, then collect the money through whatever method your society already uses.
Should I ask for student ID or date of birth?
Usually not. Year of study and email are enough for most societies. If you run trips or activities with health or age requirements, you are responsible for what you collect, so ask only for what you need and explain why.
Can I see which activities people are most interested in?
Yes. Responses feed a results page with charts for each question, and you can download everything as a CSV for a spreadsheet. Choice questions are the quickest way to see which sessions to schedule.
How do I word the consent question so it is clear?
Say exactly what they are agreeing to, such as receiving emails about sessions and events, and that they can unsubscribe at any time. A separate tick box for each kind of message is clearer than one box for everything.
