This is a real form: answer it the way a visitor would. Nothing is saved.

More to start from: all form, quiz and course templates. Nothing is published until you press Publish, and every answer lands in your account with charts and a CSV.

Parents are often asked one vague question at the end of term, or nothing at all, so a school learns little about its teaching, its messages or how safe families feel. This page builds a short survey you send by link or QR code, then read the answers in one results page with charts and a CSV. The live sample above is a general customer survey; the chat rewrites it around your school or kindergarten.

What you get

Follow-up questions only when needed

Logic shows a question only when an earlier answer matches. A parent who says their child does not feel safe at drop-off can be asked what happened and whether it was reported, while everyone else skips those questions and the survey stays short.

Logic and pages

Show a follow-up only when it applies, split a long form into pages, or show one question per screen.

Results you can read

A chart for every question, averages and NPS for scales, the order of a ranking, a table of every answer and a CSV.

How it works

  1. Step 1

Try the sample above, then press Use this template, or describe your own form to the chat.

  2. Step 2

Change any question by hand or ask the assistant ("add a question about price", "translate it to Spanish"), with a live preview.

  3. Step 3

Publish, share the link or QR code, and read the answers as they arrive. You get an email for each one.

Who uses it

  • Kindergarten or nursery director

Sends one survey to every family at the end of the year, then compares answers on communication and daily routine across rooms or age groups before the next intake.

  • Primary school class teacher

Asks parents of one class how they rate homework, reading support and updates, and uses the single 'one thing to change' answer to plan the next term.

  • After-school club or holiday camp coordinator

Checks whether pick-up times, activities and staff contact worked for families who only use the club part of the year, and gets a clear list of what to fix.

Questions

Should the survey ask for parents' names?

That is your choice. You can leave out a name or email field so answers are more candid, but say clearly in the introduction what you will do with the answers. Each response can also be sent to you by email if you want to follow up on a specific concern.

Can I keep the survey to a few minutes?

Yes. Ask for six to ten questions, use rating scales and multiple choice for most of them, and keep one open comment box at the end. Parents answer more of the survey when it is short and clearly structured.

How do I send it to all families?

Publish the survey to get a link and a QR code, then post them in your messaging app, on the notice board or in the pick-up area. You can set a response limit if you only want a certain number of answers.

What is my responsibility for the answers?

You are responsible for what you collect about families and children, and this page cannot make that decision for you. Ask only what you need to improve the school, avoid asking for children's personal details in the survey, and check local rules on data before you publish.

Can I change questions after the chat drafts them?

Yes. Ask the assistant to reword a question, add a choice, remove a block or move the safety questions earlier, and it will edit the survey in the chat. Then review the final version before you publish.
