How do you build an anonymous employee engagement survey for a small team?

Build an anonymous employee engagement survey in chat: workload, management, growth and an eNPS question, with results you can chart and export as CSV.

One quick question

Not at all likelyExtremely likely

This is a real form: answer it the way a visitor would. Nothing is saved.

More to start from: all form, quiz and course templates. Nothing is published until you press Publish, and every answer lands in your account with charts and a CSV.

Small teams often run an engagement survey once, get vague answers, and then nothing changes, so people stop replying. This page helps you write a survey that asks about the specific things your team is dealing with, such as workload, how managers respond, and growth, and that makes the results easy to read. The live sample above is a general NPS survey; the chat adapts it to your company, your questions and your wording.

What you get

Scale blocks with a results page you can read and export

Each workload, management and growth question can be a 1 to 5 scale, and the eNPS question is a 0 to 10 scale. The results page charts how answers are spread, and the CSV lets you compare this quarter with the next one without rebuilding anything.

Logic and pages

Show a follow-up only when it applies, split a long form into pages, or show one question per screen.

Results you can read

A chart for every question, averages and NPS for scales, the order of a ranking, a table of every answer and a CSV.

How it works

  1. Step 1

Try the sample above, then press Use this template, or describe your own form to the chat.

  2. Step 2

Change any question by hand or ask the assistant ("add a question about price", "translate it to Spanish"), with a live preview.

  3. Step 3

Publish, share the link or QR code, and read the answers as they arrive. You get an email for each one.

Who uses it

  • A founder with 8 to 15 staff

You want honest feedback on workload and management without the answers being traced back to people. The chat keeps the survey to about ten questions, so people finish it, and leaves out name and email fields.

  • An office manager running the survey for the first time

You need questions that are easy to explain and a page that works on a phone. Ask for the team size and the decisions you could realistically make, and the chat will shape the wording to match.

  • A team lead who wants to follow up on one department

You want the results broken down without identifying anyone. Describe your teams and the chat can add a question about team, but keep the groups large enough that no one can be picked out from a single answer.

Questions

Can an employee survey really be anonymous in a small company?

Only if the questions don't identify anyone. With five people, asking for department plus tenure plus role can point to one person even without a name. Leave out identifying questions and only report results for groups of several people.

What should an eNPS question look like?

The standard form asks how likely someone is to recommend the company as a place to work, on a 0 to 10 scale, followed by one question about the reason. The score is the share of 9s and 10s minus the share of 0 to 6s, so keep the wording exactly as it is for comparison.

Should I send the survey by email and ask people to reply with their name?

No. Put the survey at a link or QR code, don't ask for a name or email, and keep the open comment optional. Each response can send you an email, so turn that off if it would make people feel watched.

How long should the survey be?

Most people will finish about ten questions in a few minutes. Longer surveys get rushed answers and more blank comment boxes, so focus on what you can actually act on in the next quarter.

What do I need to be careful about when collecting staff feedback?

You are responsible for what you collect from your staff, so ask only what you need to act on and say plainly what you will do with the results. Check the local rules on employee data before you publish the survey.
