A restaurant feedback survey guests can finish at the table

Build a short guest survey for your restaurant or cafe: food, service, wait, value and return visits. Open it from a table QR code.

1 = poor, 5 = excellent

This is a real form: answer it the way a visitor would. Nothing is saved.

More to start from: all form, quiz and course templates. Nothing is published until you press Publish, and every answer lands in your account with charts and a CSV.

Guests who have a poor meal usually pay and leave without saying why, and a star rating on a review site does not tell you whether the kitchen was slow or the plates were cold. This page makes a survey that asks the specific questions, so you can see which part to fix. The live sample above is a general customer satisfaction survey; the chat rewrites it for your menu, your service style and the questions you care about.

What you get

Follow-up questions only after a low score

Logic shows a question only when an earlier answer matches. Set the wait-time or 'what went wrong' question to appear only after a guest rates food or service 1 or 2, so guests who had a good meal are not asked to explain a problem they did not have.

Logic and pages

Show a follow-up only when it applies, split a long form into pages, or show one question per screen.

Results you can read

A chart for every question, averages and NPS for scales, the order of a ranking, a table of every answer and a CSV.

How it works

  1. Step 1

Try the sample above, then press Use this template, or describe your own form to the chat.

  2. Step 2

Change any question by hand or ask the assistant ("add a question about price", "translate it to Spanish"), with a live preview.

  3. Step 3

Publish, share the link or QR code, and read the answers as they arrive. You get an email for each one.

Who uses it

  • A small cafe owner

Wants to know whether the morning queue is the real complaint or whether the coffee is. Asking about wait time separately from service shows which one is driving low scores.

  • A restaurant manager

Reviews the results page each week to compare food and service scores, then exports the CSV to check whether the weekend crowd is rating the place differently from weekday guests.

  • An owner opening a second location

Makes a separate survey for each site with its own link and QR code, so the results for the two kitchens and dining rooms stay apart.

Questions

Where should I put the QR code?

The page gives you a link and a QR code. Print it on table tents, the receipt or a card near the till, so guests can scan it with their phone before they leave.

How long should a table survey be?

Keep it to six to eight questions. A guest who is still sitting down will finish it in under a minute, and short surveys get more complete answers. The chat can cut yours down if it runs long.

Can I get an email for each response?

Yes. The survey can send you an email for each response. If you want to reply to unhappy guests, add an optional email question and say clearly what you will use it for.

Will this replace review sites?

No. It gives guests a private place to explain a problem before they post publicly, so you hear about a slow kitchen or a cold dish from the guest directly and can respond to it.

Can I stop collecting responses after a set number?

Yes, a response limit closes the survey once it reaches the number you set. That is useful for a promotion or a one-week test at a single location.
