A salon booking request form you confirm yourself
Build a salon booking request form: service, preferred days, stylist, hair length and allergies. Share the link and confirm each request yourself.
Register for the event

This is a real form: answer it the way a visitor would. Nothing is saved.
Use this template Describe yours in chatOpen full screenMore to start from: all form, quiz and course templates. Nothing is published until you press Publish, and every answer lands in your account with charts and a CSV.
Many salons take booking requests through DMs, texts or phone messages, then lose track of which stylist, day or allergy was mentioned. This page builds a request form that collects those details in one place, so you can confirm or decline each request yourself. The live sample above is a general event registration form; the chat adapts it to your salon's services, stylists and hours.
What you get
Questions that appear only when they apply
Logic shows a question only when an earlier answer matches. Colour and bleach questions can appear only when a colour or highlight service is picked, so a simple cut request stays short for the client.
Logic and pages
Show a follow-up only when it applies, split a long form into pages, or show one question per screen.
Results you can read
A chart for every question, averages and NPS for scales, the order of a ranking, a table of every answer and a CSV.
How it works
- Step 1
Try the sample above, then press Use this template, or describe your own form to the chat.
- Step 2
Change any question by hand or ask the assistant ("add a question about price", "translate it to Spanish"), with a live preview.
- Step 3
Publish, share the link or QR code, and read the answers as they arrive. You get an email for each one.
Who uses it
A solo stylist renting a chair
You are often with a client when a request arrives. The form collects the service, days and hair length in one message, so you can read them between appointments and reply when you have a free slot.
A salon with several stylists
The stylist question tells you who a client wants before you check that person's diary. Requests for a specific stylist can be sent to them directly, and the rest go to whoever is free.
A brow, lash or nail studio
Patch-test and tint-allergy questions come up often in this kind of work. The form asks for them at the start, so you know whether a service needs a patch test before you confirm the booking.
Questions
Does the form book the appointment for the client?
No. It collects a request with the service, preferred days and times, stylist and hair length. You then confirm or decline by phone or email. There is no calendar or booking slot in the form.
Can I ask clients about allergies?
Yes, but you are responsible for what you collect and how you store it, so ask only what you need to choose a safe service, such as dye, bleach or product allergies. Keep the wording simple and tell clients why you ask.
Can clients send a photo of their hair or a reference picture?
No. The form has no file upload field. Ask clients to describe the look in words or bring a photo to the appointment.
How do I stop requests when my week is full?
Use the response limit to close the form after a set number of requests. You can also turn off the link when you no longer want new requests.
How do I see the requests?
You get an email for each response, and the results page shows all requests with a CSV download, so you can sort them by day, stylist or service.
