Make a parent-teacher meeting request form for your class or school

Build a parent-teacher meeting request form in chat: child, class, preferred time slots and topics. Share the link and get each request by email.

Register for the event

Saturday 14 June, 10:00 to 16:00. Places are limited.

This is a real form: answer it the way a visitor would. Nothing is saved.

Use this template Describe yours in chatOpen full screen

More to start from: all form, quiz and course templates. Nothing is published until you press Publish, and every answer lands in your account with charts and a CSV.

Arranging a parent-teacher meeting by email usually turns into a long thread: which child, which teacher, which of five possible times, and what the worry is. A request form collects all of that once, in the same order, so you can see the options before you reply. The live sample above is a general event registration; the chat adapts it into a meeting request for your class, your time slots and the topics you actually handle.

What you get

Ranked time preferences

Parents rank the time slots you offer, first to last, instead of choosing just one. Every request's order shows in the results and the CSV, and you confirm the slot yourself by email. The form does not hold or book times, so the ranking is your input, not a calendar.

Logic and pages

Show a follow-up only when it applies, split a long form into pages, or show one question per screen.

Results you can read

A chart for every question, averages and NPS for scales, the order of a ranking, a table of every answer and a CSV.

How it works

  1. Step 1

Try the sample above, then press Use this template, or describe your own form to the chat.

  2. Step 2

Change any question by hand or ask the assistant ("add a question about price", "translate it to Spanish"), with a live preview.

  3. Step 3

Publish, share the link or QR code, and read the answers as they arrive. You get an email for each one.

Who uses it

  • Primary class teachers

You get a short written note on what each parent wants to talk about before a ten-minute slot, so you can prepare the right student work and avoid a second meeting.

  • Secondary form tutors and heads of year

Parents with several children can request a meeting for each one. A logic rule shows the subject-concern question only when a parent picks a specific subject, so the form stays short for everyone else.

  • School offices and support staff

The office gets requests for meetings with a SENCO or pastoral lead in one place, with a consent question and a preference for call or email. Only the details needed to arrange the meeting are requested.

Questions

Can the form book the meeting in my calendar?

No. The form has no calendar or booking step. Parents send their ranked preferences, you read each one in the results or the CSV, and you confirm a time by email or your usual system.

Can I stop requests once my slots are full?

You can set a response limit, but it caps the total number of responses, not the number for each time slot. A simpler approach is to close the form or change the slot options when you have no free times left, then confirm the rest by email.

What if a parent has two children at the school?

Add a yes/no question asking whether they want to discuss another child. Logic can then show a second set of child and class fields only when the answer is yes, so single-child families see a short form.

What should I avoid asking on a meeting request?

You are responsible for what you collect about children and families, so ask only for what you need to arrange the meeting and discuss the topic. Diagnoses, medical records and family circumstances are usually better handled in the meeting itself or through the school's official process.

Can parents attach a report or a letter?

No. The form has no file upload field. Ask parents to describe the concern briefly, and say they can send any documents by email to the school office.
