Induction and refresher training often ends with a signed sheet nobody can check, so you cannot tell who understood the fire exit rules or the lifting technique. This page builds a quiz with a pass mark and records each person's name and score so you can see who passed and which questions they missed. The live sample above is a general one; the chat rewrites it around your site, equipment and rules.

What you get

Named results with a pass mark

Each answer is linked to the name the employee typed, with their score and the questions they got wrong. The results page shows the outcomes in one place, and you can export them as a CSV for your training records.

Quizzes that teach

Every question can have a hint and an explanation, and missed questions come back until they are right.

See who knows what

The results show the average score, which questions people miss most, the most common wrong answer and every play.

How it works

  1. Step 1

Play the sample above to see the hints, explanations and the practice round.

  2. Step 2

Press Use this template, or tell the chat a topic, or attach a PDF, Word file, book or YouTube link to build the questions from.

  3. Step 3

Edit any question by hand, publish, and share the link. Scores and weak spots appear in your account.

Who uses it

  • Site manager onboarding new starters

Builds a quiz on the hazards of your own site, such as pedestrian routes, machine guarding and the emergency assembly point, so new staff answer about the place they will work.

  • Shift supervisor after a near miss

Writes a short refresher on the one procedure that failed, asks for names, and shows which people on the shift have not yet answered the questions correctly.

  • Contractor coordinator for agency staff

Creates a site safety check that agency workers complete before their first shift, with the pass mark set by you and results sent to your email for each person.

Questions

Can the quiz require a name before people start?

Yes. The name is a question you place first, so the employee types it before the safety questions, and that name is attached to their score on the results page.

Does a pass mark here count as an official certificate?

No. The quiz records scores and answers for your own files. Check what your regulator or insurer accepts as proof of training, and keep any certificate they require alongside this record.

Who can see the names and scores?

You see them on the results page and in the CSV. The quiz is opened by a link, so anyone with that link can take it. Share it only with the staff who need to sit the test.

What personal details should I ask for?

You are responsible for what you collect. Ask for a name, team or site and the answers you need to assess the training, and avoid health details or other personal data the test does not need.

Can I change questions for different roles?

Yes. Logic can show a question only when an earlier answer matches, so you can send forklift drivers to driving questions and office staff to the questions that fit their work.
