Net Promoter Score is one question, answered from 0 to 10, plus a reason. Promoters score 9 or 10, detractors 0 to 6, and the score is promoters minus detractors in percent. The form above is exactly that, one question per screen, and the results page calculates your NPS and shows the distribution so you can see whether it comes from a few angry users or many lukewarm ones.

What is a good NPS?
It depends on the sector. Anything above zero means more promoters than detractors; above 30 is usually considered good, above 50 excellent.
Why ask for the reason?
The number tells you where you stand; the reason tells you what to change.
