Build a fitness class sign up form for your gym, yoga or running group

Build a fitness class sign up form for a gym, yoga or running group. Ask class, day, experience and injuries, add a waiver, get replies by email.

Register for the event

Saturday 14 June, 10:00 to 16:00. Places are limited.

This is a real form: answer it the way a visitor would. Nothing is saved.

Use this template Describe yours in chatOpen full screen

More to start from: all form, quiz and course templates. Nothing is published until you press Publish, and every answer lands in your account with charts and a CSV.

A sign-up list looks simple until the first session, when you need to know who is a complete beginner, who has a bad knee, and whether the 7am class still has room. This page builds a sign-up form for your class in the chat, with the questions, waiver consent and places limit set for your case. The live sample above is a general event registration form; the chat adapts it to your class.

What you get

Show a question only when it matters

Logic can show the injury description box only to people who answer Yes to 'any injury or condition the instructor should know about'. Most people skip it, the instructor reads the answers that need attention, and the group is not asked for detail nobody needs.

Logic and pages

Show a follow-up only when it applies, split a long form into pages, or show one question per screen.

Results you can read

A chart for every question, averages and NPS for scales, the order of a ranking, a table of every answer and a CSV.

How it works

  1. Step 1

Try the sample above, then press Use this template, or describe your own form to the chat.

  2. Step 2

Change any question by hand or ask the assistant ("add a question about price", "translate it to Spanish"), with a live preview.

  3. Step 3

Publish, share the link or QR code, and read the answers as they arrive. You get an email for each one.

Who uses it

  • A yoga teacher with a weekly community class

A form with a fixed number of places, a class and day choice, and an experience question, so beginners and regulars can be placed in the right session before the first lesson.

  • A running club leader for a Sunday long run

A form that asks for usual weekly distance and pace, the meeting point they will come to, and a consent block for the club's waiver, with a thank-you message repeating the start time and what to bring.

  • A gym running a six-week strength course

A form with experience and current activity questions, a yes/no on injuries that opens a short description only when needed, an emergency contact, and an email for each response so the coach can check the list.

Questions

Do I need a waiver, and does the form write it for me?

The form does not write a waiver. You can add a consent block where people tick that they have read your waiver text and accept the risks. The wording depends on your country, your activity and your insurer, so check it with them or a solicitor before you publish.

How do I stop sign-ups when the class is full?

Set a response limit on the form to the number of places in that session. When the limit is reached, the form stops accepting new entries. If you run several sessions, make one form per session so each has its own limit.

Should I ask about injuries and medical conditions?

Only ask what helps you run the class safely. You are responsible for what you collect about health, so a yes/no question with an optional short description is usually enough. Do not ask for diagnoses or medical records, and tell people how you will store their answers.

Where do the answers go?

Each response can be emailed to you, and the results page shows answers with charts and a CSV download. You can export the list to a spreadsheet for check-in on the day. Keep that file private and delete it once the course ends if you no longer need it.

Can people pay for the class through the form?

No. The form has no payment step, so take payment separately, for example by bank transfer or at the door, and use the thank-you message to say how to pay and where to meet.
