Build a client intake questionnaire that tells you what a new project needs before the first call

Make a client intake questionnaire for your freelance or agency work. Ask about goals, budget range, timeline, and who signs off, then get each reply by email.

We usually answer within a day.

This is a real form: answer it the way a visitor would. Nothing is saved.

More to start from: all form, quiz and course templates. Nothing is published until you press Publish, and every answer lands in your account with charts and a CSV.

Most freelance and agency projects go wrong in the first two weeks, when the client's goals, budget and approver are still unclear. A client intake questionnaire gets those answers in writing before the first call, so you can say no early or quote accurately. The live sample above is a general contact form; the chat turns it into a questionnaire built for your kind of client and your services.

What you get

Show a question only when an earlier answer calls for it

Logic lets you show the approver questions only when a client says someone else signs off, and show the detailed budget step only for services you actually sell. Clients answer fewer irrelevant questions, and every reply you get is one you need.

Logic and pages

Show a follow-up only when it applies, split a long form into pages, or show one question per screen.

Results you can read

A chart for every question, averages and NPS for scales, the order of a ranking, a table of every answer and a CSV.

How it works

  1. Step 1

Try the sample above, then press Use this template, or describe your own form to the chat.

  2. Step 2

Change any question by hand or ask the assistant ("add a question about price", "translate it to Spanish"), with a live preview.

  3. Step 3

Publish, share the link or QR code, and read the answers as they arrive. You get an email for each one.

Who uses it

  • Freelance designers and developers

Ask for goals, example sites and a budget range before quoting, so your proposal matches what the client expects to spend and how they want it to look.

  • Small agencies with several services

Use choice questions to route each enquiry to the right service, and add a question on who approves the final work so you do not spend weeks on a proposal the client cannot sign off.

  • Consultants and copywriters taking on new clients

Collect the timeline, audience and current materials in one place, then decide which leads fit your schedule before you book a discovery call.

Questions

Can clients upload their brand files or documents through the questionnaire?

No. There is no file upload field. Ask clients to paste links to their shared folder, website or examples in a text question, and agree on a file transfer method for after the project starts.

Should I ask for the budget range in the questionnaire?

Usually yes, but as ranges with a 'not sure yet' option, not as a single figure. Clients are more willing to answer a range, and you still get enough information to decide whether to quote.

How do I find out whether the person I'm talking to can approve the work?

Ask directly who makes the final decision and whether anyone else needs to approve it. If a second person is involved, a follow-up question can show their name and email, so you know who to include in the proposal.

Where do the answers go, and can I see them all together?

Each response can be sent to your email as it arrives. The results page shows all responses with charts and can be exported as a CSV, so you can compare leads by budget or timeline.

Can I use the questionnaire for different services with different questions?

Yes. Build one questionnaire with shared questions and conditional follow-ups for each service, or make a separate link for each service. The chat can set this up for your case and you can edit it at any time.
